Submission of this form does not guarantee acceptance. Sundown Audio reserves the right to request items to be returned through the standard RMA procedure at your expense.

Only items that are 2yrs old or under may approved. Only items that have serial numbers may be approved. Ask your account manager for special requests.

For speakers, you must provide a picture of the product damaged beyond repair before a FD# can be granted. For subwoofers, No picture is required.
